  • We fix errors and note what changed.

Who we are

TransplantCheck is made by Mental Wealth Solutions. Patient words, shared check-ins and transplant follow-up for dialysis social work teams.

What is this guide for?

This guide is for patients, families and dialysis teams in New York, New Jersey and Connecticut. Use it to find sources and plan what to ask your team.

How we check facts

  • We use official sources only.
  • We check each fact word for word against the source.
  • We show the date we checked it.
  • When we cannot check something, we leave it out.

Our review schedule: check center reports each January and July, federal payment rules each fall, transplant policy when it changes, and center pages every three months.

What we don’t do

  • We don’t rank centers.
  • We don’t give advice about one person’s care.
  • We don’t track visitors.
  • We don’t use patient stories or names in this guide.

How do I report a mistake?

Email us about the page and the error. Please leave out patient names and health details. We fix errors and note the change on the page.

Questions to bring to your team

  • Which source supports this answer?
  • When was this page checked?
  • What does this mean for my care?
  • Who should I ask for help?
  • What should I bring to our next talk?
